  1. NCD follow-up

    Who it serves: people with hypertension, diabetes or kidney disease. Example KPIs: logging rate, medication adherence, appointment attendance.

  2. VHV and sub-district hospital support

    Who it serves: citizens in the catchment area. Example KPIs: automated check-ins, better-targeted home visits.

  3. Older adults

    Who it serves: adults aged 60+. Example KPIs: daily check-in response, timely escalation.

  4. Screening and health literacy

    Who it serves: at-risk groups in the screening plan. Example KPIs: screening uptake, post-screening follow-up.

  5. Mental wellbeing

    Who it serves: the general population. Example KPIs: mood check-ins, referrals to 1323.
